vancomycine,
laaddosis 25 mg/kg (max. 2,5 g), toegediend over minstens 1 tot 2 uur (< 15 mg/min).
Start na de laaddosis onmiddellijk een continu infuus aan 30 mg/kg/24h (max. 4 g/dag)
