Klasse 4: ernstige infectie
Elke lidmaat bedreigende voetinfectie met 2 of meer van van de volgende symptomen:
- temperatuur >38°C of <36°C
- hartslag >90sl/min
- ademhalingsfrequentie >20/min of PaCO2 <4.3KPa (32mmHg)
- witte bloed cellen >12 10^9L, of >10% onrijpe neutrofielen

Amoxicilline-clavulaanzuur 1 g/200 mg q6h iv

Ciprofloxacine
500 mg q12h po of 400 mg q12h iv
Bij IgE gemedieerde penicilline allergie:
Clindamycine
600 mg q8h po of iv
en
Ciprofloxacine
500 mg q12h po of 400 mg q12h iv
Recente behandeling (<4 weken) met amoxicilline-clavulaanzuur of ciprofloxacine
Piperacilline/tazobactam
4 g/500 mg q6h iv
Indien ESBL-positieve kiemen in wonde
Meropenem
1-2 g q8h iv
